NAACOS RECENT COMMENTS
IPPS Proposed Rule. NAACOS comments on the 2026 Inpatient Prospective Payment Systems (IPPS) proposed rule centered on the proposed changes to the Transforming Episode Accountability Model (TEAM) and the request for information (RFI) on transitioning to digital quality measurement (dQM).
Medicare Deregulation. NAACOS response to the CMS RFI on opportunities to streamline regulations and reduce administrative burdens in the Medicare program highlighted recommended regulatory changes to ease the burden of quality reporting, simplify beneficiary notifications, and improve benchmark approaches.

SENATE SEEKS CHANGES TO HOUSE-PASSED RECONCILIATION BILL
The House-passed reconciliation bill remains a flashpoint in the Senate, where conservative and moderate Republicans remain at odds on several key policies. Moderates are pushing back against proposed Medicaid and other safety net cuts, while fiscal conservatives are alarmed by the bill’s projected increases in federal deficits and debt. Some GOP senators are again considering cuts to Medicare Advantage, eyeing overpayments as potential sources of waste, fraud, and abuse.
Impact on Accountable Care. The House-passed version of the reconciliation bill would eliminate Medicare’s differential conversion factor update that includes a higher update for clinicians in advanced alternative payment models (APMs) and provides a flat inflationary update to all clinicians. While inflationary adjustments are needed, this proposal removes financial incentives for clinicians to participate in risk-bearing APMs, undermining years of progress in value-based care. NAACOS is asking Republican lawmakers on the Senate Finance Committee to include a multi-year extension of advanced APM incentives to address this issue.
Uncertain Outlook Ahead. President Trump, Speaker Johnson, and Majority Leader Thune are pressing for final passage by July 4—a timeline many still consider unrealistic. The bill’s path forward remains uncertain amid rising tensions.

PRESIDENT’S BUDGET PROPOSES STEEP CUTS TO HEALTH PROGRAMS
The White House recently released additional details about the President’s Fiscal Year 2026 proposed budget, including justifications for CMS programs. The budget proposes significant cuts across public health programs, including staff reductions and restructuring. Notably, the President is proposing to move the 340B program from HRSA to CMS.
Impact on Accountable Care. While the budget proposes steep cuts to many programs across HHS, it continues to invest in CMS’ value-based health care programs. The CMS Innovation Center also released a strategy in May that outlines the Administration’s plans to help Americans live healthier lives.
Focus on Fraud, Waste and Abuse. The budged increased investments to combat fraud, waste, and abuse in Medicare and Medicaid. NAACOS has been engaging with the Administration on opportunities to better partner with ACOs to address fraud, waste, and abuse.
Next Steps. Presidential budgets do not carry the force of law since funding needs to be approved by Congress. We expect congressional Republicans to begin drafting spending bills that more closely align with the President’s budget request. This will set up a drawn-out political battle between Republicans and Democrats ahead of the September 30 government funding deadline.

CMS released the first round of participation and performance data for Performance Year (PY) 2023 of the Quality Payment Program (QPP), including both Merit-based Incentive Payment System (MIPS) and APM participation. The snapshot provides aggregated data for key QPP program metrics, including a high-level overview of participation, scoring, and payment adjustment information.

MIDWEST REGIONAL MEETING SCHEDULED AUGUST 15, INDIANAPOLIS
Registration is now open for the August 15 Midwest Regional Meeting. Our regional events are open to NAACOS ACO members, non-members, and value-based care providers. These meetings have been created to provide a space for discussion inclusive of the full care continuum including ACOs, CINS, provider practices, hospitals, home health, skilled nursing facilities, hospice, community-based organizations, and more. The agenda is being developed to gain a deeper understanding of the successes and the challenges that still exist in population health care evolution and contracting that meets the needs of health care organizations and patient populations. The session topics for this event include Medicaid and D-SNPS, advocacy and the impact of national legislation on local regions, post-acute collaboration to meet the needs of value-based care focused organizations and using AI to improve outcomes in VBC. Each NAACOS member will receive one free seat to an in-person meeting, per membership, per year. Additional seats are $250 to attend the live events, and registration is open and can be accessed on the Midwest Regional Meeting Registration webpage.
