This session presents the one-year results of a value-based care initiative combining informatics and community health workers to support patients through this transition, achieving a 50% reduction in unplanned admissions, and $1.45M in total cost of care savings. In this session, we will share how data-driven variation analysis uncovered actionable opportunities across the continuum of care, sparked deeper clinical review of high-cost and high-variation episodes, and ultimately informed FY27 strategic initiatives. Attendees will learn how our organization moved from identifying variation to developing targeted interventions to improve outcomes, reduce unnecessary utilization, and prepare for emerging episode-based payment models.
