ACO and Community Care Hub Partnerships in the Northwest: Real-World Examples, Results, Federal Investments, and Significant Opportunities

The US Administration for Community Living will briefly highlight why scaling partnerships between community-based organizations and health care partners such as ACOs through community care hubs (CCH) is a high priority, as well as current and upcoming federal investments to support this work. Then, two advanced CCHs (Oregon Wellness Network and the Denver Regional Council of Governments) will discuss their partnerships with ACOs and other health care partners, including degree of collaboration, populations serve, impact achieved, and the business case. The audience will have a chance to engage with the speakers.

This presentation will describe how the Midwest QIN-QIO can augment and support Quality Improvement efforts in outpatient clinical settings, and will provide a case study of how this support can help providers with quality reporting, including integration of data into EMR systems, and make sustainable improvements in quality of care for patients.
