From Fragmentation to Integration – Care Coordination for Recurrent Rural Health Care Users Envoy Integrated Health’s journey from a small clinically integrated network launched with 5 clinics in 2021 to a physician-led ACO now spanning 40 clinics and more than 7,000 Medicare beneficiaries, using a decentralized, low-overhead model to support local care management, data, and quality work. The session will highlight development of a community care hub linking clinics and community-based organizations and explore how Alaska’s Rural Health Transformation Program can catalyze deeper integration with rural facilities for high-need, high-utilizing patients. |
