This session examines how Acute Care at Home models of care can be deployed as a proactive, patient-centered model of care in an ACO. We will connect clinical design choices with the operational infrastructure needed to deliver hospital-level services reliably in the home. Share lessons learned on required clinical capabilities, patient criteria, and return on investment. Participants will also explore where Acute Care at Home fits in the ACO continuum, the performance measures that matter, and the financial and contracting considerations that determine sustainability under value-based arrangements.

Advancing High-Value Care Through Clinical Excellence
We will hear from Boston Medical Center Health System on their initiative to reduce rates of unplanned dialysis among patients with advancing chronic kidney disease (CKD).
We will also learn about BREATHE (Better Breathing and Reduced Exacerbations through Education, Adherence, Titration, Health Navigation, and Empowerment), a pharmacy-led intervention designed to reduce acute exacerbations of COPD and asthma.
