MAHA ELEVATE Overview

The CMS Innovation Center (CMMI) has released the MAHA ELEVATE (Make America Healthy Again: Enhancing Lifestyle and Evaluating Value‑based Approaches Through Evidence) Model Notice of Funding Opportunity. This model focuses on lifestyle medicine approaches, chronic disease prevention, and psychological or self-care strategies to support whole‑person care. 

Key Features of the MAHA ELEVATE Model: 

Goals: 

  • Test evidence‑based lifestyle, nutrition, stress management, and preventive interventions that are currently non-covered services for Traditional Medicare beneficiaries 
  • Proposals should bring credible evidence and specify how interventions complement existing care, particularly for chronic disease management and prevention 
  • Gather and evaluate data on cost and quality to inform future Medicare coverage determinations and potential future CMMI models 

Eligibility: 

  • Organizations participating in the Medicare Shared Savings Program are eligible to apply. Organizations accepted to multiple active CMS Innovation Center models will be assessed for participation on a case-by-case basis. 

Award: 

  • $100M in federal cooperative agreement funding over 3 years 
  • Up to 30 awardees, including health systems, ACOs, provider groups, FQHC AND RHCs, and community-based organizations 

Timeline: 

  • Applicants must submit a mandatory Letter of Intent by April 10, 2026 
  • Applications for the first cohort of recipients are due on May 15, 2026 
  • Model will launch in October 2026
