ACO and Community Care Hub Partnerships in the Southwest: Real-world Examples, Results, Federal Investments, and Significant Opportunities

The US Administration for Community Living will briefly highlight why scaling partnerships between community-based organizations and health care partners such as ACOs through community care hubs (CCH) is a high priority, as well as current and upcoming federal investments to support this work. Then, an advanced CCH (the Partners in Care Foundation) will discuss their partnerships with ACOs and other health care partners, including degree of collaboration, populations served, impact achieved, and the business case. The audience will have a chance to engage with the speakers.
