Advancing Collaboration and Shared Learning in the ACCESS Model

The ACCESS Coalition is a collaborative multi-sector forum designed to accelerate evidence-based technology-enabled care through the CMS Innovation Center’s ACCESS (Advancing Chronic Care with Effective, Scalable Solutions) Model.

The coalition will serve as a trusted forum for constructive problem-solving and discussion on operational and policy issues affecting the ACCESS Model and technology-enabled accountable care. Participants will be able to share common insights about implementation, lessons learned, and practical approaches to strengthening care delivery, coordination, and model success. The Coalition may also identify opportunities for model improvement to CMS and other policymakers to support the continued evolution of the ACCESS Model and value-based care.

Who Participates?

The Coalition engages the organizations participating directly in the ACCESS Model and their partners in the value-based care ecosystem, including accountable care organizations (ACOs); physician groups and practices and health systems, along with other clinicians and their care teams; and other health technology and innovation partners.
