NAACOS Honors 2026 Excellence Award Recipients
Bryan Health Connect ACO, LLC, Ochsner Accountable Care Network, and Tandigm Value Partners recognized for their outstanding commitment and success in accountable care
WASHINGTON (October 15, 2026) – The National Association of Accountable Care Organizations (NAACOS) has recognized Bryan Health Connect ACO, LLC (BHC), Ochsner Accountable Care Network, and TriValley Primary Care, as a participant of Tandigm Value Partners ACO, with its 2026 Excellence Awards. The organizations were honored at NAACOS’ Fall Conference for their outstanding commitment to and accomplishments in accountable care across different lines of business and payers.
“The true impact of accountable care can be obscured by quality scores, savings, and performance metrics. Behind those results are healthier patients, supported clinicians, stronger communities, and a more sustainable health care system. Our Excellence Award recipients demonstrate what is possible when accountable care becomes a core organizational strategy. Through innovation, leadership, and a commitment to better outcomes, these organizations are creating meaningful change that extends far beyond Medicare and serves as a model for the future of health care,” said Emily Brower, president and chief executive officer of NAACOS.
The three awardees represent the diverse ways accountable care strategies can be used to meet the needs of the patients and communities they serve.
Bryan Health Connect ACO, LLC, has built a scalable, technology-enabled infrastructure that integrates real-time data, care coordination, and partner engagement across its value-based care network to unite more than 3,000 providers serving 110,000 patients in value-based care arrangements across all lines of business in Nebraska. By leveraging advanced analytics, automated patient transition alerts, and strong collaboration with post-acute providers, the organization has achieved sustained improvements in quality, utilization, and total cost of care. These innovations have reduced administrative burden, strengthened care transitions, and enabled more proactive, patient-centered care for the populations it serves.
Ochsner Accountable Care Network serves more than 60,000 Medicare beneficiaries and more than 535,000 patient lives across all lines of business through more than 3,500 clinicians. Their population-health model operates in Louisiana, Mississippi, Alabama, Florida, Georgia, and South Carolina to integrate clinical care teams, post-acute providers, and community organizations around patient needs. Ochsner leverages advanced technology, coordinated care management, and community partnerships to improve preventive care, chronic disease outcomes, and patient experience while reducing costs across government and commercial contracts. Together, these efforts help patients stay healthier, support clinicians in delivering more coordinated care, and strengthen the communities OACN serves.
TriValley Primary Care, a participant in Tandigm Value Partners ACO, cares for more than 9,000 Medicare beneficiaries in southeastern Pennsylvania. Ithas distinguished itself through a scalable, team-based approach to value-based care that combines physician leadership, care management, and population health expertise across practice sites. Its commitment to innovation and continuous improvement has driven Medicare Shared Savings Program performance from 6.4% in 2022 to more than 13% in 2024, while sustaining strong quality performance. Strategic partnerships with continuing care retirement communities and the adoption of AI-enabled technology have strengthened care coordination, reduced administrative burden, and improved outcomes for older adults. While TriValley participated in Tandigm Value Partners ACO, MSSP performance increased from 6.4% in 2022 to more than 13% in 2024 for 11,500 Medicare beneficiaries.
The Excellence Awards, started in 2024, recognize high-performing organizations that have demonstrated outstanding commitment to and accomplishments in value-based care. Recipients of the award exemplify how value-based care can transform health care delivery by improving quality and outcomes, lowering or slowing growth of costs, and increasing patient and provider satisfaction. Through their ongoing commitment to fostering collaboration and accountability in health care, the honored organizations demonstrate NAACOS’ mission of advancing provider-led transformation toward better care, improved health, and smarter spending.
