ACO REACH

The ACO Realizing Equity, Access, and Community Health (REACH) Model builds on the successful Next Generation ACO Model to offer higher levels of risk and reward than the Medicare Shared Savings Program. ACO REACH replaced the Global and Professional Direct Contracting Model on January 1, 2023 and is currently set to expire at the end of 2026. ACO REACH offers a heightened focus on health equity, improvements to risk adjustment, lower and discounts than GPDC. NAACOS provides specialized resources, education, and advocacy for our ACO REACH members.
