Annual Wellness Visits– Proactive Primary Care Chaired by Jen Perloff, Institute for Accountable Care and Kevin McKeill, Lehigh Valley Health Network ACO
This session is designed to cover the basics of annual wellness visits (AWVs) including the required elements of a visit, engaging primary care providers, outreach and engaging Medicare beneficiaries, adaptations to the electronic health record and following up on the results of screenings. We will also talk about the clinical value of these visits, particularly for multi-morbid, older or frail beneficiaries. The session will be co-facilitated by Jennifer Perloff from the Institute for Accountable Care and Dr. Kevin McKeill from the Lehigh Valley Health Network ACO with presentations from ACOs that have developed comprehensive AWV strategies in their ACOs. Participants should leave the session with tools and techniques for setting up a structured AWV process, including insights on operational challenges and solutions from ACO experts.
